On 13 April 2007, 51 year old truck driver William McMaster, who was employed by Levira Pty Ltd, was delivering bread to a bakery operated by McLeod’s Traditional Pty Ltd. During the delivery he was using a forklift at the bakery to move trolleys of bread. After parking the forklift Mr McMaster was returning to his truck when the forklift rolled down a ramp and struck him, pinning him against the truck’s trailer.
