    Downloadable OHS Forms (Take 5's, Hazard Reports etc)

    My name is Nathan Smith and I thought I would come on here to introduce a new downloadable service that I am offering. I have been producing Health and Safety Forms for mining and industrial companies for about 13 years now (things like Take 5's, Hazard Reports, Incident Reports and Vehicle Pre-start Books). I have found that every now and then I receive requests from clients for generic, well designed forms that can be downloaded. We came up with www.take5safetyforms.com as a way of making our documents easily available. I have put a number of popular OHS forms in one download.     I looked at your Hazard Report. In others I seen and use for my work there are two blocks as your has for corrective actions; 1) interim controls, 2) permanent abatement. This assures the work can continue to be done with differing procedures temporarily until permanent abatement actions can be programmed... bids taken with third party contractors, new equipment procurement, etc... The other thing I didn't see was the Risk Assessment Score; your form has L, M, H hazards. If I have several ticked 'H' which one has priority for abatement first? Typically, the worst hazard is abated first based on hazard severity and likihood (duration). Is the hazardous task performed intermittently, > than one work shift per week, three work shifts per week, or throughout the week; we call these exposure points. For chemical hazards are the unprotected exposures* less than the TLV, STEL, AL= 1/2 TLV, Excursion Limit or greater and if greater, how many shifts per week and how many workers are potentially exposed? * Always discount the use of PPE when assessing hazards. PPE is a the control of last resort in the hierarchy of controls. The more workers potentially exposed for longer durations when unprotected exposures are above the triggering concentration, the greater the overall risk, ceteris paribus.
